Registration
New patients are requested to complete a registration form together with a health questionnaire, prior to seeing a doctor or a new patient health check. Patients register with the Practice, not with an individual GP. When making an appointment you may express a preference as to which doctor you would like to see.You will be asked to provide ID.
If you are an Overseas Visitor you will be seen as a private patient, unless there is a reciprocal agreement in place.
