Registration
If you would like to register with the practice, please bring two types of identification to reception. One of these should be photographic ID; the other should be a recent bank or utility statement showing your address. These will be scanned into our computer system, and you will be given a registration pack to complete and return. The pack includes health questionnaires and other information required by the Department of Health and the primary care trust.
Once the completed forms are returned, you will be invited for a new patient health check with one of the clinical team. Patients register with the Practice, not with an individual GP. When making an appointment you may express a preference as to which doctor you would like to see.
If you are an Overseas Visitor you will be seen as a private patient, unless there is a reciprocal agreement in place.
