Registration
As it takes between 7-10 days to register you, please ensure that you have enough medication from your previous GP to ensure that you don't run out.
You can register as a patient at Park View Health Partnership if you live in RH15 postcode in West Sussex and you are not registered as a patient at another practice in Burgess Hill (unless there are special circumstances).
If you would like to register with the practice, please bring two types of identification to reception. One of these should be photographic ID; the other should be a recent bank or utility statement showing your address. These will be logged for our records, and you will be given a registration pack to complete and return. The pack includes health questionnaires and other information required by the Department of Health and the primary care trust.
Once the completed forms are returned, you will be invited for a new patient health check with one of the clinical team. It would be very helpful if you could return these in person so that we can check that we have all the required information and arrange an appointment with you in person. Patients register with the Practice, not with an individual GP. When making an appointment you may express a preference as to which doctor you would like to see.
If you are an Overseas Visitor you will be seen as a private patient, unless there is a reciprocal agreement in place.
